A: Nipple reconstruction is the final stage of breast reconstruction after mastectomy. It recreates the nipple projection using local tissue flaps, and the areola using skin grafts or medical tattooing. Why Nipple Reconstruction Matters
For many women, breast reconstruction feels truly complete only after nipple and areola restoration. It is a relatively minor procedure — usually performed under local anaesthesia — but its psychological impact is profound. It is always the patient's choice and can be done at any time after the primary reconstruction has fully healed.
Stage 1: Nipple Projection
A small local skin flap is raised from the reconstructed breast mound to create the nipple projection. Common techniques include the Star Flap, C-V Flap, and skate flap — all of which use local tissue and require no additional donor site.
Stage 2: Areola Reconstruction
The areola is recreated using one of two methods:
- Skin graft from the inner thigh or groin (which has similar natural pigmentation to areola skin)
- Medical tattooing (3D nipple tattoo) — increasingly preferred as it avoids an additional donor site and produces remarkably realistic, natural-looking results

- Typically performed 3–6 months after the primary breast reconstruction
